In the past penis extenders used a silicone noose type system. Whilst this has always been an effective method and continues to work for many users extenders have now moved on. Many extenders now use a comfort strap system. For the end user this means you are able to wear the extender for longer periods without feeling any discomfort that could sometimes be felt with the noose type extenders.
